Our After School Program is a charming blend of creativity, positive friendships, and enriching activities designed to make every school day end with sparkle. At Little Princess Spa®, we provide a loving, safe environment where children can unwind, play, and participate in themed experiences designed to build confidence, kindness, and fun.

Parents can relax knowing their child is in great hands. Our afternoons blend imaginative spa-style fun with crafts, learning, and self-expression. Please review the following information carefully before registering.

Release of Medical Information & Emergency Authorization I authorize Little Princess Spa® to contact my child’s physician and obtain relevant information if needed. In the event of an emergency:

• Every effort will be made to contact me immediately.
• If I cannot be reached, I give permission for emergency care to be administered.
• I will arrange for pickup within one hour of notification.
• I agree to inform staff if my child has been exposed to a communicable illness.
• In urgent cases, I consent for emergency responders to transport my child if necessary.

Health Certification I certify that:

• My child is in good health and able to fully participate in program activities.
• My child has had no serious illness or operation since their last medical exam.
• My child has not had recent exposure to contagious illness.

If your child has any diagnosed medical, developmental, or behavioral conditions (e.g., ADHD, Autism, anxiety), please describe or attach documentation to help us best support your child.

Parent Agreement By enrolling my child, I agree to the following:

• My child may participate in all scheduled program activities.
• I understand this is a group setting and individual tutoring or 1-on-1 homework assistance is not provided.
• Staff offer basic supervision during homework time, but it is not guaranteed my child will complete assignments.
• Little Princess Spa® is not responsible for incomplete or missing homework.
• Full payment must be submitted by 6:00 PM on the Monday prior to the start of each new billing cycle.
• All supplies and materials are purchased based on registration numbers; therefore, no refunds will be issued unless accompanied by a licensed physician’s note.
• I understand that in the event of an emergency and I cannot be reached, staff will act in the best interest of my child.
• My child is fully potty trained.
• My child will follow program rules and behave respectfully. If removed due to behavior concerns, no refunds will be issued.
• My child may appear in promotional photos and videos. I give permission for Little Princess Spa® to use their image without compensation.
• I will label all personal belongings. Little Princess Spa® is not responsible for lost or damaged items.
• The program ends promptly at 5:30 PM. I understand the late fee applies for pickups after 5:30 PM.
• My child will only be released to individuals listed on the authorized pickup form who present valid photo ID.
• I will notify staff of any changes to my child’s health, schedule, or pickup list.
• I agree to sign my child in and out daily.

Program Days & Hours Weekdays Only – Monday through Friday • Pickup by 6:00 PM We follow the school year calendar and are closed on national holidays and select school breaks. Daily schedule includes:

• After school arrival
• Snack time
• Homework period (supervised only)
• Themed enrichment activities
• Spa play, arts, movement & more!

Program Fee Options Weekly • Monthly • Semester Plans All fees are non-refundable and non-transferable. Auto-payment is required to hold your child’s spot.

Camper Age & Readiness Ages: 5  to 12 years old All children must be fully potty trained.

Snacks & Water Parents must pack a healthy afternoon snack and a labeled reusable water bottle daily. We are a nut-free facility. Please avoid packing items that contain peanut or tree nuts.

Extra Clothes Please pack an emergency change of clothes labeled in a Ziplock bag.

Electronic Devices We strongly recommend that children leave electronic devices at home. Our goal is to promote social interaction and imaginative play.
